Is Your Client a Good Fit?
Before submitting a referral, please confirm your client meets the following placement criteria:
Ambulatory and mobile without assistance
Independent or semi-independent in daily activities
Cognitively aware and oriented
Has a verified and guaranteed income source
Does not require major medical care, behavioral stabilization, or 24-hour supervision
